Blood cancers, also known as hematologic cancers, are life-threatening diseases that affect the production and function of blood cells. These cancers can originate in the bone marrow, lymphatic system, or blood itself. Timely and accurate diagnosis, as well as staging, play a vital role in determining the best course of treatment and prognosis for individuals experiencing these diseases. Since September is observed as Blood Cancer Awareness Month, Dr. Gaurav Dixit, Unit Head – Hemato Oncology (Unit II), Artemis Hospital Gurgaon Essential aspects of blood cancer diagnosis and staging are explained.

Symptoms and physical examination

Blood cancer can manifest with a variety of symptoms, including fatigue, unexplained weight loss, frequent infections, bruising, and swollen lymph nodes. Initial diagnosis often begins with a thorough physical examination by a health care professional. The health care professional will assess your medical history and symptoms to determine the need for further testing.

blood test

Blood tests are a fundamental part of the blood cancer diagnostic process. A complete blood count (CBC) measures the levels of various blood components, including red blood cells, white blood cells, and platelets. Abnormalities in these counts can provide important information about the presence of blood cancer.

bone marrow biopsy

To confirm the diagnosis and determine the specific type of blood cancer, a bone marrow biopsy is often needed. During this procedure, a small sample of bone marrow is collected and examined under a microscope. This helps in identifying the type and stage of blood cancer.

Staging is an important step in understanding the extent and progression of blood cancer. It guides treatment decisions and provides information about the patient’s prognosis. Blood cancer staging is usually done using systems that are specific to each type of cancer, but they usually share some common elements. Let us understand each step briefly.

step 0

This is the initial stage. At this stage, the cancer is confined to its original site and has not spread to nearby tissues or organs. It is considered localized.

Stage I

At this stage, the cancer is still confined to its original location, but may begin to affect nearby tissues or lymph nodes.

Phase II

At this stage, the cancer has usually spread to nearby lymph nodes, but not to distant parts of the body.

Phase III

At this stage, the cancer has progressed, affecting multiple lymph nodes or tissues within the area.

Stage IV

This is advanced stage. At this stage, the cancer has spread to distant organs or tissues, such as the liver, lungs, or bone marrow.

Staging is further refined with letters (A or B) and numbers (0, 1, 2) to provide more precise information about the extent of the disease. For example, stage IIA would indicate somewhat more advanced but still localized cancer than stage IIB.

Diagnosis and staging of blood cancer are important steps in determining the appropriate treatment plan and predicting the patient’s outlook. These procedures depend on a combination of symptoms, physical examinations, blood tests, and, in many cases, a bone marrow biopsy. Understanding the stage of blood cancer is important for both patients and healthcare providers to make informed decisions about treatment options and to monitor disease progression.

It is important to remember that advances in medical research and technology continue to improve our ability to effectively diagnose and treat blood cancers. Regular screening and early detection can have a significant impact on the patient’s prognosis. It is also important to highlight the importance of awareness and regular health care visits for people at risk or experiencing blood cancer-related symptoms so that they can be diagnosed early and treatment can be started before the condition worsens.
